Patient Terms & Conditions

General Terms for All Patients

• Please provide at least 24 hours' notice if you need to cancel an appointment. Short-notice cancellations or missed appointments reduce availability for other patients.
• If you cancel with less than 24 hours' notice or miss more than two appointments, we may be unable to continue offering treatment.
• Any patient who is violent or abusive toward staff, other patients, or anyone on the premises will be refused treatment and reported to the appropriate authorities.
• All patients must have a completed medical history form before treatment.
  • If you have not completed the form online, please arrive 15 minutes before your appointment to complete it at the practice.
• Please allow plenty of time to arrive for your appointment. If you are late, we may not be able to see you, and this will be recorded as a missed appointment.

NHS Patients

• Please provide your NHS number.
• If you are claiming exemption from NHS charges, please bring valid proof of your exemption.
• NHS charges must be paid upfront before treatment begins.

Private Patients (this also includes private treatment for NHS or Denplan patients)

• A deposit is required when booking private appointments.
• We reserve the right to charge a fee for missed appointments or cancellations made at short notice.

Denplan Patients

• Denplan does not cover missed or short-notice cancellations.
• We therefore reserve the right to charge a fee for these appointments.
