Terms & Conditions for this offer:

To claim, just mention the 10% offer when booking your appointment.
Offer can be withdrawn at any time and is at the discretion of Charles Street Dental Surgery.
Only applies to the initial private examination fee.
Can only be claimed once per person, and only by people who have not previously been a patient of the Practice.
Cannot be used in conjunction with any other offer.
A deposit will be taken for the appointment at time of booking. If the appointment is missed, or cancelled with less than 24 hours' notice, this will be retained by the practice in line with our policy on missed appointments.

Your Responsibilities

We require 24 hours' notice for cancelling an appointment. Missing appointments wastes time and resources which are needed for other patients. If you cancel with less than 24 hours' notice or miss an appointment on more than two occasions, then we may no longer be able to complete or offer NHS treatment in the future.
We reserve the right to charge a fee for missed private/Denplan appointments.
Patients who are violent or abusive to practice personnel, to other patients or anyone else on the practice premises will be refused treatment and reported to the appropriate authorities.
If you are claiming exemption from NHS charges, please bring proof with you.
If you have not filled in a medical history form in the past 6 months, please arrive early enough to allow time to complete a new one prior to your appointment.

NHS Patients

If you are not currently registered with an NHS dentist, you can apply for a place online using the Dental Access Portal at cavuhb.nhs.wales.
